Complete Your Forms
These forms help us understand your health history and current conditions so we can better assess and prepare a treatment plan. Paperwork typically takes ~10 minutes to complete. Need help? Just ask! We’re happy to walk through them with you.
Evaluate Your Needs
Before we begin, our primary goal is to holistically evaluate if you may need additional services beyond what a chiropractic care can provide— such as emergency care or legal support — If this is the case, we will work with you every step of the way.
Address Your Pain
Let the pain relief begin! Dr. Tony and Dr. Theresa will implement a custom treatment plan aimed at reducing or even eliminating your issue. From there, we will determine how many sessions you’ll need to complete your care.

- No-Fault Insurance
If you’ve been injured in a car accident, your costs are most likely fully covered. Not sure? We can help you figure it out.

If you’ve been injured in a work accident, your costs are fully covered under your employer’s Worker’s Compensation insurance.
- Individual Insurance
We accept Blue Cross/Blue Shield, Independent Health & Medicare insurance. Depending on your particular plan, chiropractic care may be fully covered.
- Cash
Paying this way helps us control costs and focus on you, not your insurer. Payments are as low as $40/appointment.
